If your concern relates to a safeguarding matter, appropriate information may need to be shared with relevant safeguarding, regulatory, professional or statutory bodies where required to protect people from harm.

Make a Complaint or Raise a Concern

Use this form to make a complaint, raise a safeguarding, quality or staff conduct concern, or speak up (whistleblowing). You can use this route to raise concerns about unsafe practice, wrongdoing, misconduct or other matters that you believe should be brought to our attention.
